Managing the Infertility Patient with Endometriosis

Women with endometriosis are at risk for decreased ovarian reserve due to both the pathophysiology of the disease and iatrogenic injury resulting from surgery. Fertility preservation begins with judicious decision-making about the patients most likely to benefit from surgery. Repeat surgery with the goal of improving fertility outcomes should be avoided. Long-term postoperative hormonal suppression reduces endometrioma recurrence and should be offered to patients who are not actively attempting conception. Women with decreased ovarian reserve and advanced maternal age or those who are at risk for bilateral ovarian injury, including women with bilateral endometriomas or a recurrent contralateral endometrioma, may benefit from ovarian tissue or oocyte cryopreservation prior to surgery. In this chapter, we provide practical recommendations regarding fertility preservation and sequencing surgery and in vitro fertilization in women with endometriosis.
