Bone Marrow Stem Cell Chemotactic Activity Is Induced by Elevated CXCl12 in Endometriosis

This paper investigated CXCL12 (SDF-1) and CXCR4 expression and function in endometriotic lesions, using endometriosis biopsies from women undergoing laparoscopy and eutopic endometrium from controls, alongside cultured endometriosis-conditioned media. CXCL12 and CXCR4 mRNA were increased in endometriosis versus control, CXCR4 protein was higher in all lesion compartments, and CXCL12 protein was highest in the epithelial compartment; conditioned media from cultured endometriosis further increased CXCL12 levels by ELISA. Using a Transwell migration assay, the authors found a two-fold increase in chemoattraction of mouse bone marrow stem cells toward CXCL12-enriched endometriosis-conditioned medium compared with eutopic endometrium. A key limitation is that functional recruitment was tested in a mouse system and does not directly measure stem cell recruitment in human patients. This paper is centrally about endometriosis — it links elevated CXCL12/CXCR4 signaling in endometriotic lesions to increased chemotaxis of bone marrow stem cells.
