Abstract

The management of endometriosis and its impact on fertility is a complex and multifaceted challenge that requires a comprehensive understanding of the disease process, its relationship with fertility, and the available treatment options. Although oral contraceptive pills, progestins, and GnRH agonists and antagonists can exhibit considerable efficacy in alleviating endometriosis symptoms both before and after pregnancy, pretreatment with these agents does not appear to enhance fertility. Conservative surgery can have fertility benefits, but fertility preservation prior to surgery should always be considered given the risk of iatrogenic harm to ovarian reserve. IVF is the mainstay of fertility treatment in the setting of endometriosis. Ongoing research, fortified by recent increases in federal funding and attention, aims to enhance our understanding of endometriosis, with a specific focus on improving diagnostic methods, treatment modalities, and fertility outcomes. The mechanisms underpinning infertility associated with endometriosis remain a crucial area for exploration, informing the development of targeted therapies and interventions.
