This study employed immunohistochemistry to characterize sympathetic and sensory nerve fibers in the uteri of 40 women with adenomyosis compared to a control group of 20 women without endometriosis or adenomyosis. The results indicated that while large myelinated nerves were less frequent in the myometrium of adenomyosis patients, there was a significant reduction in sympathetic tyrosine hydroxylase-positive nerves within this tissue layer. Conversely, small unmyelinated fibers and substance P-positive sensory nerves were more prevalent near adenomyosis lesions, suggesting a neurotrophic environment that favors sensory innervation over sympathetic innervation. This paper is centrally about adenomyosis — specifically investigating the differential distribution of autonomic and sensory nerve fibers within the affected uterine tissue to understand pain mechanisms.
