The patient presented with an irregular menstrual cycle and abnormal genital bleeding that progressed to severe anemia, and MRI showed endometrial hyperplasia with a myometrial lesion featuring a high-signal multicystic mass, leading to a confusing preoperative diagnosis that simulated infiltrating malignancy. After hysterectomy, gross examination demonstrated villous endometrium and a thickened myometrium with multiple small cysts containing serous transparent fluid, and the final diagnosis was adenomyosis coexisting with simple endometrial hyperplasia. As a case report, the major limitation is that conclusions are based on one patient’s imaging and pathology findings.
