This paper reviews chronic pelvic pain (CPP) in menstruating persons, focusing on endometriosis-associated chronic pelvic pain (endo-CPP), and discusses its pathophysiology and clinical heterogeneity across superficial/deep lesions, endometriomas, and multisystem symptoms. Drawing on prior evidence, it highlights that endometriosis-related pain is multi-factorial, involving lesion-associated inflammation and pain mediators as well as nociceptive, neuropathic, and nociplastic mechanisms such as peripheral, central, and cross-organ sensitization, which may help explain variable symptom intensity and persistence despite hormonal therapy or lesion excision; it also notes CPP occurs in about one-third of women even after surgical/hormonal treatment. A limitation acknowledged by the authors is the lack of a non-invasive diagnostic standard, with diagnosis often relying on surgical evaluation or imaging that may be constrained by access. This paper is centrally about endometriosis — it focuses specifically on mechanisms and presentation of endometriosis-associated chronic pelvic pain (endo-CPP).
