Abstract
Deep dyspareunia affects 50% of women with endometriosis and has significant negative effects on relationships and sexual function. Previous research has identified painful bladder syndrome as a significant contributor to deep dyspareunia which suggests factors such as central sensitization that may contribute to the etiology of deep dyspareunia. To determine the relationship between central sensitization, painful bladder syndrome, and severity of deep dyspareunia in women with endometriosis. We hypothesized that a greater level of central sensitization will be found in the group with high deep dyspareunia and painful bladder syndrome, compared to the other groups. Prospective registry at an endometriosis tertiary centre. Primary outcome: central sensitization measured using the self-reported Central Sensitization Inventory (0-100) questionnaire. Variables of interest: painful bladder syndrome (yes/no) assessed using diagnostic criteria; and severity of deep dyspareunia measured from 0-10 and categorized as low (<5) and high (≥5). Inclusion: new or re-referred to the centre between January-June 2018; 18-50 years. Exclusion: never sexually active; post-menopausal. Bivariate relationships between the primary outcomes and variables of interest were examined. The sample was divided into groups for further analysis; Group 1 had a low severity of deep dyspareunia and no painful bladder syndrome; Group 2 had a low severity of deep dyspareunia and painful bladder syndrome; Group 3 had a high severity of deep dyspareunia and no painful bladder syndrome; Group 4 had a high severity of deep dyspareunia and painful bladder syndrome.
