Disclosing the invisible : experiences, outcomes and quality of endometriosis healthcare

This thesis synthesized four studies to characterize experiences, encounters, and outcomes of endometriosis healthcare from the perspectives of nine women with laparoscopy-verified endometriosis (qualitative interviews) and 25 healthcare professionals (qualitative interviews), alongside quantitative comparisons in women with persistent pelvic pain using quantitative sensory testing, questionnaires for health-related quality of life and anxiety/depression, and register-based patient-reported outcomes after benign hysterectomy. Women and healthcare professionals described difficulty disclosing and communicating hidden pain, professional insecurity and limited knowledge, and the diagnostic pathway as central to understanding and enduring persistent pelvic pain; in women with persistent pelvic pain, reduced pain thresholds, lower quality of life, and higher anxiety/depressive symptoms were observed, with pain-threshold reductions correlating with longer symptom duration. However, women with persistent pelvic pain with versus without an endometriosis diagnosis did not show significant differences in pain thresholds or psychosocial outcomes in the quantitative study, and overall hysterectomy outcomes were largely positive in the benign indication cohort.
