目的探讨卵巢子宫内膜异位症(endometriosis,EM)瘤变的临床特点、超声图像特征及预后。方法回顾性分析23例卵巢EM瘤变的临床、超声及病理资料,随访其预后。结果卵巢EM恶变的高危因素有年龄〉45岁,特别是绝经后女性,异常阴道流血、腹痛性质改变,超声提示包块短期内迅速增大、囊肿直径〉8cm、囊壁见乳头或血流丰富。随访Ⅰ期患者16例均未见复发;Ⅲ~Ⅳ期患者3例均死亡。结论联合病史和经阴道彩色多普勒超声检查是早发现卵巢EM恶变的有效方法。
